ABOUT THE PROJECT

Restore Hope Liberia is implementing the FHI 360 EpiC Maternal, Newborn and Child Health (MNCH) Continuity of Care Project as a Civil Society Organization (CSO) subawardee under the USAID/PEPFAR-supported EpiC platform.

The project seeks to strengthen continuity of care across the maternal, newborn and child health continuum through improved pregnancy identification, referral completion, antenatal care continuity, facility delivery support, postpartum follow-up, newborn follow-up, PMTCT/EID continuity, immunization follow-up, maternal wellbeing support, and strengthened community-facility linkages.

POSITION SUMMARY

The Maternal Wellbeing & Continuity Facilitator will serve as a frontline community-based implementation officer responsible for strengthening continuity of care between households, communities, and health facilities.

The Facilitator will work closely with Community Health Assistants (CHAs), Trained Traditional Midwives (TTMs), Community Health Services Supervisors (CHSS), health facilities, community leaders, and families to support pregnancy identification, referral completion, follow-up activities, maternal wellbeing support, and continuity of care throughout the maternal, newborn, and child health continuum.

The position will play a critical role in ensuring that women, newborns, and children remain connected to appropriate services and support systems.

PROJECT STAFFING PHILOSOPHY

KEY RESPONSIBILITIES

  1. Pregnancy Identification and Tracking
  • Support identification and registration of pregnant women.
    • Assist with pregnancy mapping activities.
    • Promote early ANC enrollment.
    • Support referral tracking and follow-up.
  1. ANC Continuity and Referral Support
  • Promote ANC attendance and continuity.
    • Support referral completion and follow-up.
    • Assist women in navigating referral pathways.
    • Identify barriers to service utilization and support solutions.
  1. Facility Delivery and Birth Preparedness Support
  • Promote facility delivery.
    • Support birth preparedness planning.
    • Assist with transport readiness discussions.
    • Strengthen communication between communities and facilities.
  1. Postpartum and Newborn Follow-Up
  • Support follow-up of postpartum mothers and newborns.
    • Promote postnatal care attendance.
    • Monitor continuity of care after delivery.
    • Support referral and follow-up for identified concerns.
  1. PMTCT/EID and Immunization Continuity
  • Support follow-up of mother-baby pairs.
    • Promote immunization attendance and completion.
    • Assist with referral verification and follow-up.
    • Support tracing of missed appointments where appropriate.
  1. Maternal Wellbeing and Psychosocial Support
  • Support maternal wellbeing screening activities.
    • Provide basic psychosocial support and referral.
    • Promote participation in approved maternal wellbeing activities.
    • Identify and refer women requiring additional support.
  1. Community Engagement and Dialogue
  • Support community dialogue activities.
    • Engage community leaders, women’s groups, youth groups, CHAs, TTMs, and other stakeholders.
    • Promote awareness of MNCH continuity issues.
    • Strengthen community ownership of referral and continuity systems.
  1. Documentation and Reporting
  • Maintain activity records and referral documentation.
    • Complete required project forms and tools.
    • Submit timely reports to supervisors.
    • Support collection of implementation evidence and success stories.

QUALIFICATIONS AND EXPERIENCE

Required Qualifications

  • Diploma in Nursing, Midwifery, Community Health, Public Health, Social Work, Social Sciences, or a related field; or equivalent community health experience.
    • Minimum two (2) years of experience working in community health, MNCH, public health, social services, or community development.
    • Experience working with communities and vulnerable populations.
    • Strong communication and interpersonal skills.
    • Ability to work independently in field settings.
    • Willingness to travel extensively within assigned communities.

Preferred Qualifications

  • Experience working with CHAs, TTMs, CHSS, health facilities, or community health programs.
    • Experience supporting MNCH, maternal health, mental health, psychosocial support, or referral systems.
    • Experience supporting donor-funded projects.
    • Familiarity with assigned implementation areas.
