As part of the implementation of its “STRIDES Project” funded by the United States Department of States, FHI 360 is requesting a quotation for the below goods (s): Laboratory Reagents and Consumables for MOHLiberia

1. Proposal Requirements/Quotation Format.

The Vendor’s proposal shall include the following:

• The solicitation numbers
• The name, address, and telephone number of the vendor
• The date submitted
• Detailed vehicle specifications
• Quote Validity
• Bill To: FHI 360 Office – Liberia
• Currency of Quote – United States Dollars
• Tax Information clearly stated on quote
• Delivery Terms
• Vendors terms and conditions
• Payment terms – Clearly state payment terms
• Warranty terms / Manufacturer’s warranty

1. Basis for Award of Contact or Purchase Order –The quotations will be evaluated based on the following criteria:
   • Competitive pricing -20%
   • Capacity to supply (ISO 9001 2015 certification) – 20%
   • Source/Origin – 10%
   • References – 10%
   • Delivery time – 10%
   • Warranty – 10%
   • Payment Terms – 10%
   • Ability to meet technical specification – 10%

1. Purchase Order Terms:
   • This purchase order (PO) is subject to all the terms and conditions outlined in FHI 360

PO Terms “Supplier’s delivery of products, the performance of services or issuance of invoices in connection with this purchase order establishes Supplier’s agreement to the Terms. The Terms may be modified only in writing and signed by both parties.

• Termination: FHI 360 has the right to terminate the contract at anytime
• This contract shall not be subcontracted to a 3rd Party.
